I agree that match officials may record my participation, shirt number, goals and any cards issued in the official match report, and that the league may retain that record.
I release the league, the host club and the venue from liability for injury or loss arising from my participation, except where caused by negligence on their part.
This acknowledgement covers every match I play in the 2026 season and stays in force until the season ends or I withdraw it in writing.
A copy is emailed to you. Questions go to your club, not to FIH.
